Key Responsibilities\n

• Conduct thorough assessments and ongoing evaluations of patients to tailor individualized treatment plans effectively.

• Communicate consistently with supervisors, interdisciplinary team members, and family members regarding patient progress and therapeutic strategies.

• Manage scheduling and coordination of appointments, home visits, and progress tracking to ensure comprehensive treatment continuity.

• Engage in home visits to perform observations, conduct chart reviews, and analyze behavioral data, as necessary for holistic patient assessment.

• Facilitate the implementation of therapeutic programs, ensuring adherence to established care plans.

• Participate actively in Interdisciplinary Team Meetings, providing insights and collaboration for integrated care approaches.

• Document and maintain thorough monthly and quarterly progress notes to reflect patient development and plan adjustments as needed.

• Organize and lead training sessions to ensure that staff and team members are proficient in program implementation and documentation protocols.

• Provide follow-up support through additional interviews and home visits to adapt treatment strategies based on evolving client needs.
