Key Responsibilities\n

• Conduct thorough assessments of patients' mental health needs, accommodating various modalities for engagement.

• Maintain regular and open communication with supervisors, interdisciplinary team members, families, and guardians regarding patient progress and treatment plans.

• Prepare tentative schedules for appointments, home visits, and ongoing evaluations of patient care.
\n
• Coordinate and execute home visits, including conducting chart reviews and interviews to ensure all treatment strategies are effectively applied.
\n
• Monitor implementation of individualized treatment plans and adjust as necessary to enhance patient outcomes.
\n
• Participate actively in Interdisciplinary Team Meetings to contribute insights and collaborate on care approaches.
\n
• Complete and document monthly and quarterly progress notes to track patient development.
\n
• Equip team members with training required for the proficient implementation of treatment plans and documentation processes.
\n
• Facilitate follow-up interviews and home visits to provide continued support and adjustments based on individual patient needs.

As a Licensed Mental Health Therapist, you will engage with a diverse array of clients, including adults and youth facing challenges such as anxiety, depression, trauma, substance misuse, and family dynamics. Qualifications\n

• A Master’s or Doctorate degree in a related field is required.

• Current and unrestricted licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW) in California.

• Minimum of 2 years of relevant counseling experience, ideally in a clinical environment.

• A strong understanding of ethical guidelines and legal standards governing mental health practice.
