Work Environment

This full-time position is primarily onsite, emphasizing one-on-one interactions with patients in their homes. You will work Monday through Friday, collaborating with a diverse team of healthcare professionals and utilizing evidence-based practices to provide the highest standard of care. This environment requires adaptability and creativity, as each patient's home is unique and may present different challenges and opportunities for rehabilitation.

Key Responsibilities

• Develop and implement individualized therapy plans that target specific rehabilitation goals, focusing on preventing disability, alleviating pain, and restoring muscle function.
• Oversee and collaborate with Physical Therapy Assistants to ensure the effective delivery of therapeutic interventions, adhering to the established plan of care.
• Conduct thorough evaluations of clients' physical capabilities, employing diagnostic tests for muscles, nerves, joints, and functional abilities.
• Assess clients' home environments and provide actionable recommendations aimed at enhancing their independence and safety.
• Administer a variety of therapeutic techniques, including manual exercises, traction, and the application of physical agents, tailored to each patient's needs.
• Evaluate, fit, and adjust prosthetic and orthotic devices, coordinating with physicians for necessary modifications.
• Document and maintain accurate clinical records, including initial evaluations, care plans, and progress notes, in compliance with regulatory and agency standards.
• Participate in interdisciplinary case conferences to facilitate comprehensive treatment planning and evaluation.
• Educate clients and their families on the use and care of physical therapy devices as well as the overall rehabilitation program.
• Engage in ongoing assessments to modify therapy plans based on clients' progress and changing needs.
• Complete additional responsibilities as assigned to support the overall goals of the organization.

Patient Population / Client Focus

In this role, you will work primarily with a diverse population of clients, each with unique rehabilitation needs. Your patients may include individuals recovering from surgery, managing chronic pain, or working towards regaining functionality after an injury. Providing compassionate, adaptive care in their familiar environments not only enhances their comfort but also motivates them to engage actively in their recovery journey.

Qualifications

• Graduate of an accredited physical therapy program, holding a Bachelor's Degree or higher.
• A minimum of one (1) year of experience as a physical therapist in acute care or home care settings.
• Active Physical Therapy licensure in good standing in the state of California.
• Current CPR certification to ensure client safety during treatment.
• A valid driver's license with personal automobile insurance, as travel to client homes is required.
• A strong commitment to clinical excellence, quality outcomes, and integrity in practice.
• Ability to make informed decisions in challenging situations while taking responsibility for patient outcomes.

Preferred Experience

• Experience working in home healthcare settings, particularly with geriatric populations or individuals with complex medical needs.
• Familiarity with OASIS documentation and other regulatory standards related to home health therapy.
• Proficiency in using electronic health records (EHR) systems for efficient and accurate documentation.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 100,000 to 120,000, commensurate with experience and qualifications.
