Key Responsibilities

• Conduct thorough evaluations, including reviewing relevant patient history and performing tailored assessments to establish treatment plans.
• Clearly communicate evaluation findings and treatment plans to patients and their families, actively involving them in goal setting.
• Implement customized treatment protocols based on individual evaluations, patient presentation, and best practices in the evolving healthcare landscape.
• Consult and coordinate with other healthcare professionals as needed, ensuring comprehensive patient care.
• Document changes in patients' conditions meticulously, adjusting treatment plans appropriately and ensuring compliance with care protocols.
• Manage discharge planning efficiently, ensuring that patients receive appropriate follow-up care post-rehabilitation.
• Maintain precise documentation of patient interactions and care activities, including legible daily notes, interim progress summaries, and final evaluations.
• Showcase excellent time management skills by adhering to productivity expectations and maintaining an accurate schedule.
• Participate in community outreach initiatives and marketing activities aimed at raising awareness of our services.

Patient Population / Client Focus
