Key Responsibilities

• Deliver high-quality, compassionate patient care in an outpatient setting, ensuring safety and comfort for all individuals.

• Implement and adapt treatment programs for patients based on personalized plans developed collaboratively with fellow therapists.

• Conduct thorough patient evaluations, including gathering relevant health history, performing specialized assessments, and formulating precise recommendations and documentation.

• Oversee appropriate discharge planning and follow-up care, ensuring accurate documentation of patient interactions, including initial evaluations, daily encounter notes, and progress summaries.

• Maintain compliance with corporate, clinical, and state regulations through comprehensive training and adherence to best practice standards.

• Engage in respectful and open communication with colleagues, physicians, patients, and family members to foster a collaborative care environment.

• Participate actively in team meetings, contributing to discussions aimed at enhancing facility operations and patient care protocols.

Patient Population / Client Focus

Qualifications

• Possess a Doctorate degree in Physical Therapy from an accredited institution.

• Strong interpersonal skills with the ability to build rapport with patients and staff members alike.

Compensation and Benefits

Trails Physical Therapy, LLC offers a competitive salary ranging from $100,000 to $120,000 annually, commensurate with experience.
