Work Environment

This position is based in a home care setting, emphasizing a personalized approach to therapy. You will typically work collaboratively with a small team consisting of two other physical therapists and one physical therapist assistant (PTA). Our facility spans 5,000 square feet and is equipped with state-of-the-art technology to support evidence-based therapy practices. Key Responsibilities

• Conduct thorough assessments to identify the needs and rehabilitation goals of each patient.
• Provide treatment for 2-3 patients per hour, delivering high-quality, individualized care based on the best available evidence.
• Develop and implement tailored care plans incorporating various therapeutic modalities to promote physical recovery.
• Encourage active participation in rehabilitation, focusing on improving strength, flexibility, balance, and coordination.
• Utilize manual therapy techniques effectively, enhancing the overall treatment experience.
• Create and promote individualized fitness and wellness regimens that address the unique needs of each patient.
• Maintain accurate records of patient care and progress using EMR software such as WebPT.
• Work relentlessly to reduce reliance on prescription medications by exploring and implementing non-surgical treatment options.
• Collaborate with other medical professionals to ensure integrated care and optimal patient outcomes.
• Assess treatment outcomes and modify plans as necessary while consistently tracking and documenting advancements.

In this role, you will primarily serve patients recovering from orthopedic conditions, sports injuries, and other mobility-related ailments. Our team utilizes evidence-based practice to offer specialized rehabilitation services, ensuring that our patients receive tailored support to help them achieve their personal recovery goals. Qualifications

• Possession of a Doctorate in Physical Therapy (DPT).
• Active state Physical Therapist License with no board or disciplinary actions.
• Proficient manual therapy skills, demonstrating expertise in hands-on techniques.
• Familiarity with electronic medical record (EMR) systems, particularly WebPT.

Preferred Experience

• Previous experience in orthopedic outpatient care is highly desirable.
• A history of successfully developing individual treatment plans and coordinating care across disciplines.
• Experience working within a home care setting, enhancing service delivery models for improved patient satisfaction.

Compensation and Benefits
