Experienced Physical Therapist for Home Care Services

Position Overview

High Level Rehab & Therapy Clinic is actively seeking a dedicated and skilled Physical Therapist to join our home care team in Homestead, Florida. This full-time position offers a salary range of USD 90,000 – 120,000 annually, along with a comprehensive benefits package. Work Environment

This role is based onsite within the homes of patients across the Homestead area. As a part of our home care services, Physical Therapists work autonomously while maintaining constant communication with our clinic's healthcare team. Your day-to-day will involve traveling to various locations, adapting treatment plans to the home environment, and ensuring the highest standard of care is maintained at all times. Our supportive administrative staff will assist with scheduling and logistics to allow you to focus entirely on patient care.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities, assessing mobility, strength, and functional performance specifically tailored to orthopedic needs.
• Develop and implement individualized treatment plans in collaboration with referring physicians, ensuring adherence to agency policies and best practices.
• Administer high-quality physical therapy services, which include therapeutic exercises, manual therapy techniques, and modalities for pain relief.
• Educate patients and families regarding in-home treatment options, therapeutic exercises, and self-management strategies to facilitate recovery.
• Communicate effectively with other healthcare professionals involved in the patients' care to provide a coordinated and seamless treatment experience.
• Document all patient interactions and progress in accordance with regulatory standards and clinic policies.
• Continuously evaluate and adjust treatment plans based on patient progress and feedback, prioritizing outcomes that enhance their overall health and wellbeing.

Patient Population / Client Focus

Our physical therapy services concentrate primarily on orthopedic rehabilitation, including but not limited to post-surgical rehabilitation, sports injuries, arthritis management, and chronic pain conditions. The patient population consists of diverse individuals, ranging from the elderly requiring mobility and strength recovery to active adults needing rehabilitation for sports-related injuries. In this setting, you will play a pivotal role in helping patients navigate their recovery journey in the comfort of their own homes.

Qualifications

• Master's degree from an accredited physical therapy program.
• Current and valid Physical Therapy license in the state of Florida.
• Minimum of three years of experience in an outpatient setting with a strong focus on orthopedic conditions.
• Mandatory certifications in First Aid, AED, and CPR to ensure patient safety and care.
• Proficient in using electronic health record (EHR) systems for documentation and patient management.
• Strong communication skills to effectively educate patients and collaborate with healthcare teams.

Preferred Experience

• Previous experience working in home health care or a community setting is a plus.
• Demonstrated skills in implementing innovative therapy approaches and staying current with industry best practices.
• Proficiency in using technology and software related to physical therapy treatments and patient education.
