Work Environment

This position is onsite, providing home care services in the comfort of our patients' homes. Working from various locations within the Homestead area, you will generally operate in a flexible schedule environment from Monday to Friday, from 9:00 AM to 5:00 PM. Key Responsibilities

• Conduct detailed assessments to identify and address patients' goals and needs, tailoring treatment strategies accordingly.
• Design and implement individualized rehabilitation care plans, utilizing diverse therapeutic techniques that facilitate improvement in motion and mobility.
• Create and oversee fitness and wellness programs that align with patient-specific health goals.
• Deliver personalized interventions backed by evidence-based research to promote recovery from musculoskeletal injuries.
• Provide motivation and encouragement to patients during treatment, optimizing their potential to regain functional independence.
• Consult with interdisciplinary health professionals to enhance the patient's treatment plan and ensure integrated care.
• Continuously evaluate treatment effectiveness and make necessary adjustments, while keeping patients and their families informed of progress.

Patient Population / Client Focus

This role encompasses a diverse patient population, primarily focusing on individuals suffering from orthopedic injuries and conditions. You will collaborate with patients recovering from surgeries, managing chronic pain, and undergoing rehabilitation following accidents or injuries. Qualifications

• Education: A Master's Degree or Doctorate of Physical Therapy from an accredited program is mandatory.
• Licensure: You must hold an active State license as a Physical Therapist.
• Experience: A minimum of two years of hands-on experience in orthopedic rehabilitation is required.
• Proficient in patient assessments, treatment planning, and direct patient care.
• Strong communication skills, with the ability to establish rapport with patients, families, and healthcare team members.

Compensation and Benefits
