Work Environment

This position requires on-site work within a home care setting, where you will work closely with patients in their own living environments. Key Responsibilities

• Conduct detailed evaluations of patients by reviewing medical histories, completing physical assessments, and developing tailored treatment plans based on comprehensive findings.
• Effectively communicate evaluation results and proposed treatment plans to both patients and their family members, engaging them actively in the goal-setting process.
• Utilize current treatment protocols to implement therapy based on evaluation results and ongoing assessments, adapting practices as necessary to reflect changes in the patient's health status.
• Collaborate with a multidisciplinary team of healthcare professionals and coordinate with specialists when necessary to optimize patient care.
• Document patient care activities accurately, including initial evaluations, daily notes, and discharge summaries, ensuring compliance with industry regulations and reflecting the patient's progress.
• Manage your schedule efficiently while meeting productivity standards, being punctual and reliable for meetings, and participating in community outreach and marketing.
• Contribute to departmental goals and quality improvement initiatives while adhering to applicable laws and accreditation standards.
• Pursue continuing education opportunities proactively and mentor the next generation of physical therapists, enhancing both personal and organizational competencies.
• Prioritize customer service excellence, creating positive and accessible experiences for patients and families even in challenging situations.

Patient Population / Client Focus

Our patient population includes a diverse range of individuals requiring rehabilitation due to recent surgeries, chronic conditions, or mobility limitations. Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• A valid Physical Therapy license in the State of Florida is essential.

Preferred Experience
