Position Overview

Vitality Physio-Rehab Center is seeking a dedicated and compassionate Full-Time Home Care Physical Therapist to join our team in Cypress, Florida. Work Environment

The role will primarily take place within the homes of our clients across Cypress, Florida. Key Responsibilities

• Direct physical therapy treatment aimed at preventing disabilities, alleviating pain, and enhancing or restoring muscle function according to personalized care plans established in collaboration with physicians.
• Evaluate and diagnose patients' functional capabilities through various tests, including muscle strength, joint mobility, and functional ability assessments.
• Design and implement treatment strategies that are specific to each patient's conditions, ensuring they meet their individual rehabilitation goals.
• Assess the home environment to recommend modifications that promote patient independence and safety.
• Oversee the work of Physical Therapy Assistants and ensure adherence to established care plans.
• Provide instruction and support in therapeutic and functional exercises, adapting techniques to meet the unique needs of each client.
• Administer physical therapy modalities such as traction and massage, utilizing appropriate therapies to enhance recovery.
• Maintain detailed clinical records including initial assessments, care plans, and progress notes to ensure continuity of care.
• Work collaboratively in an interdisciplinary team to evaluate and make necessary adjustments to treatment protocols.
• Conduct on-site functional assessments and review treatment plans based on patient progress and feedback.

Patient Population / Client Focus

You will be working with a diverse clientele ranging from post-operative patients and individuals recovering from injuries to those with chronic conditions requiring ongoing physical therapy support. This role requires a high degree of empathy, as you will interact closely with patients and their families, helping them navigate their conditions and rehabilitation journeys.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• At least 1 year of professional experience in physical therapy, preferably in home care or outpatient settings.
• Current licensure as a Physical Therapist in Florida, with no disciplinary actions.
• Valid CPR certification and a commitment to maintaining up-to-date clinical skills.
• Excellent interpersonal communication skills, enabling effective engagement with patients and their families.
• A collaborative team player who is willing to lead initiatives that promote patient-centered care.
• Strong problem-solving skills and the ability to adapt treatment based on clinical observations and patient feedback.

Preferred Experience

• Experience in home health settings, particularly managing cases involving complex rehabilitation needs.
• Knowledge of healthcare regulations and standards, including OASIS assessment requirements.
• Familiarity with various therapeutic modalities and technology used in physical therapy practice.
• Previous roles involving leadership or supervision of Physical Therapy Assistants.

Compensation and Benefits

The annual salary for this position ranges between USD 100,000 and 120,000, based on experience and qualifications.
