Key Responsibilities\n

As a Licensed Professional Counselor at Oasis, your primary responsibilities will include:

• Conducting comprehensive patient intakes, which focus on diagnostic and functional evaluations, and formulating recommendations for treatment goals in collaboration with primary care providers.

• Providing brief, targeted therapeutic interventions for clients experiencing mental health challenges, ensuring evidence-based practices are utilized.

• Consulting with primary care providers to guide the management of patients’ mental health needs, determining which patients require referral to specialized mental health services.
\n
• Developing and executing relapse prevention plans to aid patients in achieving stable functioning and improved quality of life.
\n
• Monitoring patient progress, providing follow-ups, and modifying treatment approaches as needed to support optimal outcomes.

• Documenting all interactions and patient progress thoroughly within electronic health records, maintaining compliance with both agency and Federally Qualified Health Center (FQHC) standards.

• Working collaboratively with an interdisciplinary team to enhance behavioral health services and contribute to overall patient care strategies.

• Educating patients on self-management techniques and coping strategies to take an active role in their recovery journey.

Compensation and Benefits\n
