Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for hospice patients, identifying emotional, social, and financial needs to facilitate holistic care.

• Provide individual and group counseling sessions focused on grief, coping strategies, and adjustment to changes in health status, skillfully addressing the emotional challenges of terminal illness.

• Collaborate closely with a multidisciplinary team of healthcare professionals, including physicians, nurses, and chaplains, to develop and implement individualized care plans.

• Coordinate with community organizations and resources to streamline post-discharge services, ensuring continuous support for family members following a patient's passing.
