Key Responsibilities

• Conduct thorough evaluations, which involve reviewing patient histories, performing assessments, and determining individualized treatment plans while documenting all findings accurately.
• Effectively communicate evaluation results and proposed treatment plans to patients and their families, ensuring inclusivity in goal-setting and care plans.
• Implement evidence-based treatment protocols tailored to each patient's specific needs, taking into account their unique presentations and ongoing changes in their health status.
• Collaborate with other healthcare professionals to provide comprehensive care and document any changes in patient conditions that may necessitate modifications to their treatment plans.
• Develop and execute discharge plans, ensuring proper follow-up care is in place, and provide timely documentation of all patient interactions, including evaluations, daily notes, and progress summaries.
• Manage time effectively to meet productivity expectations while maintaining a flexible schedule for meetings and team activities.
• Engage in departmental goal-setting and contribute to achieving established objectives, while also complying with all relevant regulations and accreditation standards.
• Pursue continuous professional development to maintain and enhance competency, and mentor students as a clinical instructor whenever applicable.
• Promote a positive customer experience, facilitating access to care through adaptability and responsiveness to patient needs.
• Participate in community initiatives aimed at improving access to therapy services and actively engage with performance measurement tools to enhance service delivery.

Patient Population / Client Focus

Our patient demographic includes individuals recovering from surgeries, injuries, or chronic conditions, such as orthopedic injuries, neurological disorders, or post-operative rehabilitation. Our focus is on facilitating optimal recovery through tailored treatment plans that address the unique needs of every patient, allowing them to regain their independence and improve their quality of life.

Qualifications

• Must possess a Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid DPT license in the state of New Jersey is required.
• Certification in Basic Life Support (BLS) from the American Heart Association is mandatory.
• Strong interpersonal and communication skills to effectively engage with patients and collaborate with team members.

Preferred Experience

• A minimum of 2 years of direct experience in a clinical setting is preferred, although new graduates with exceptional qualifications will also be considered.
• Experience with electronic medical record systems and familiarity with rehabilitation treatment technologies will be an advantage.

Compensation and Benefits

This full-time position offers a competitive salary ranging from USD 100,000 to 120,000 annually, in addition to a comprehensive benefits package.
