Work Environment

As a Physical Therapist with us, you will work onsite, directly in our patients' homes, allowing you to provide personalized, one-on-one care. Our facility focuses on facilitating rehabilitation in the comfort of each patient's environment, enabling them to better engage with their recovery journey. Unless specified otherwise, working hours are from Monday to Friday, 8:00 am to 5:00 pm.

Key Responsibilities

• Conduct thorough assessments, evaluations, and develop customized treatment plans for patients with functional limitations and disabilities. For example, assess a patient's mobility challenges and devise a plan to enhance their walking ability.

• Ensure adherence to Florida's physical therapy practice laws while understanding the regulations that govern our profession.

• Perform comprehensive evaluations that include reviewing patient history, conducting physical assessments, and clearly documenting findings and recommendations.

• Implement treatment strategies that are tailored to the specific needs of each patient, all based on evaluation outcomes and ongoing assessment of their health status.

• Collaborate with other healthcare professionals, recognizing when changes in a patient's condition necessitate adjustments in prescribed therapies or medical consultations.

• Oversee discharge planning and continuity of care, maintaining timely documentation that includes initial evaluations, daily encounter notes, and final assessments to provide a thorough record of each patient's rehabilitation journey.

• Engage in ongoing corporate and clinical training programs to ensure compliance with local, state, and federal healthcare regulations.

• Maximize departmental productivity while ensuring that all treatments are performed with a commitment to quality patient care.

• Direct and manage treatment for all assigned patients needing medically necessary physical therapy, swiftly documenting this necessity in patient records.

• Participate in interdisciplinary meetings and conference calls, contributing insights on patient care and engaging in initiatives aimed at improving service access for all clients.

Patient Population / Client Focus

Your role is critical in assisting a diverse patient population that may include individuals recovering from injuries, surgeries, or chronic conditions. The primary demographic consists of adult patients experiencing mobility issues or pain-related limitations, necessitating tailored treatment strategies to enhance their daily function and overall well-being.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.

• A valid state license to practice physical therapy in Florida.

• Minimum of 2 years of relevant experience in physical therapy.

• Current Basic Life Support (CPR) certification.

• Exceptional interpersonal skills capable of fostering rapport with patients quickly, emphasizing empathy and understanding in treatment approaches.

Preferred Experience

• Experience in home healthcare settings is highly desirable.

• Prior involvement in multidisciplinary teams or initiatives that address barriers to care accessibility.

• Competence with digital health solutions or applications related to physical therapy may be beneficial.

Compensation and Benefits
