Key Responsibilities\n

• Provide comprehensive education and support to patients and their families regarding available community resources and post-rehabilitation services.

• Engage in discharge planning activities, ensuring smooth transitions for patients returning home or moving to alternative care settings.

Compensation and Benefits\n
