Key Responsibilities

• Develop and implement individualized treatment plans in collaboration with our interdisciplinary team, ensuring each patient's unique goals and needs are met.
• Conduct assessments and evaluations to determine patient progress, adapting treatment strategies as necessary to optimize rehabilitation outcomes.
• Educate patients and their families on therapy techniques, progressions, and the significance of their role in the healing process.
• Lead staff education sessions and facility in-services to enhance team awareness of therapy services, fostering a culture of knowledge and improvement within the facility.
• Participate in Care Plan Meetings, providing insightful updates regarding patient progress and strategies for continued improvement.
• Maintain thorough and accurate documentation of patient evaluations, plans of care, and progress notes in accordance with state regulations and clinic policies.
• Collaborate with the supervising Physical Therapist and other team members to ensure continuity of care across all levels of patient treatment.

Qualifications

• A valid PT license in good standing is required.

• Exhibit a positive attitude, demonstrating excellent interpersonal and patient engagement skills.
• Commitment to lifelong learning and maintaining proficiency with federal, state, and governing agency regulations.
• Outstanding written and oral communication skills are essential for effective patient interaction and teamwork.

Preferred Experience

• Prior experience in a Skilled Nursing Facility is highly desirable, though not mandatory.
• Competence in performing various therapeutic modalities, manual therapy techniques, and exercise prescription tailored to individual patient needs.
• A demonstrated history of collaboration and leadership, particularly in interdisciplinary team settings.
