Work Environment

This position offers a remote working setting, allowing you the flexibility to conduct your assessments and consultations from home while connecting with seniors through in-person visits as necessary. You will engage with a diverse client population through our community health clinic, collaborating with fellow professionals committed to delivering exceptional care. Our full-time work schedule runs Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours required depending on client needs.

Key Responsibilities

• Conduct initial and ongoing assessments of seniors' psychosocial needs, evaluating emotional and social factors that impact their quality of life.
• Develop personalized care plans in collaboration with clients and their families, ensuring that their specific goals and preferences are met.
• Provide counseling and emotional support to aid seniors and their families through life transitions, including health challenges, disabilities, and end-of-life issues.
• Advocate for clients' rights, facilitating access to appropriate community resources, healthcare services, and support systems.
• Work collaboratively with interdisciplinary teams of healthcare providers, such as nurses and caregivers, to create a well-rounded care strategy.
• Conduct regular follow-up visits to monitor clients' conditions, ensuring their safety and addressing any arising concerns.
• Educate clients and family caregivers on available resources, programs, and support groups that promote social engagement and enhance overall well-being.
• Maintain accurate documentation, including client assessments, care plans, and interaction notes, to comply with best practices and organizational policies.
• Stay current on social work practices, regulations, and ethical standards to ensure the highest quality service delivery.

Patient Population / Client Focus

Qualifications

• A Master's Degree in Social Work from an accredited institution.
• Valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of practical experience in a social work setting, ideally in a community or geriatric context.
• Strong understanding of social work principles, especially in relation to aging and available community resources.
• Demonstrated assessment, counseling, and problem-solving abilities.
• Exceptional interpersonal skills and the ability to develop rapport with diverse clients.
• Proficient in electronic health record (EHR) systems and standard computer applications.

Preferred Experience

• Prior experience working directly with seniors or in a community health clinic.

Compensation and Benefits
