Great Valley Psychotherapy is seeking a dedicated and skilled Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote position. The successful candidate will engage in co-occurring counseling, supporting clients facing mental health and substance use challenges within the empathetic setting of our hospice center. Key Responsibilities\n

• Assess the counseling needs of each client, employing a comprehensive and individualized approach to treatment.
\n
• Utilize the DSM (Diagnostic and Statistical Manual) criteria to perform diagnostic assessments related to substance abuse and mental health issues, determining the suitable level of care in line with ASAM (American Society of Addiction Medicine) standards.

• Develop and prepare the substance abuse counseling portion of client treatment plans, ensuring all interventions align with assessed needs.
\n
• Create discharge and continuum-of-care plans that facilitate clients' transitions to appropriate care levels, fostering sustained recovery.
\n
• Document all counseling sessions, assessments, referrals, and follow-up actions meticulously within the client’s clinical record.
\n
• Maintain an active caseload by performing ongoing case management, utilization review updates, and fulfilling treatment planning and clinical charting responsibilities as required.
\n
• Coordinate support services for both clients and their families, establishing a collaborative network that addresses each client’s unique needs.
\n
• Engage with referral sources including legal, educational, and vocational professionals to ensure that clients receive all necessary support.

• Lead interventions for crisis-oriented situations, utilizing a multi-disciplinary team framework to achieve the best outcomes for clients.

Qualifications\n

• A Master's degree in Social Work from an accredited institution, along with a current LCSW license.

Compensation and Benefits\n

The position offers a competitive salary ranging from USD 70,000 to 100,000 yearly, reflective of experience and qualifications. Our comprehensive benefits package includes:
