Work Environment

This position is located in an Assisted Living Facility where you will work directly with patients who may have complex medical histories, requiring a collaborative and strategic approach to care. Our onsite facility is equipped with modern tools and resources to support your practice, enabling streamlined communication and collaboration with other healthcare professionals, including Physicians, Registered Nurses, and support staff. This role is structured for weekdays, allowing you to maintain a healthy work-life balance with no night or weekend shifts.

Key Responsibilities

• Collaborate closely with Cardiologists to deliver high-quality care to patients.
• Conduct thorough patient assessments, including detailed medical histories and physical exams, to determine appropriate treatment plans.
• Manage chronic conditions such as Hypertension and High Cholesterol, ensuring continuous monitoring and patient education.
• Provide outpatient Electrophysiology services for patients presenting with abnormal ECGs, arrhythmias, and device management.
• Address acute cardiac conditions including Coronary Artery Disease, Congestive Heart Failure, and Stroke, offering evidence-based interventions and referrals.
• Administer and interpret stress tests, ensuring patients receive timely and appropriate follow-up care.
• Prescribe medications and therapies as per established protocols or physician guidelines.
• Educate patients and their families about cardiovascular health, treatment options, and preventive measures.
• Facilitate referrals to specialists within the practice as needed to ensure comprehensive patient care.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve a diverse patient population within our outpatient Cardiology department, addressing a range of cardiovascular health issues. Patients may include those with varying degrees of complexity, from routine follow-ups to urgent cases involving potentially life-threatening conditions. Our goal is to empower our patients through education and support, fostering a proactive approach to their cardiovascular health.

Qualifications

• Completion of a recognized advanced practice nursing program and successful completion of board certification relevant to clinical practice.
• A Master’s degree in Nursing or equivalent doctoral degree, specializing in relevant fields including Cardiology.
• A valid, current Nurse Practitioner license in the state of New York.
• Strong clinical assessment, diagnostic, and decision-making skills.
• The ability to thrive in a collaborative team environment, delivering compassionate care.

Preferred Experience

• Experience in outpatient Cardiology, including familiarity with the management of cardiac devices and electrophysiological interventions.
• Previous clinical experience managing acute and chronic cardiac conditions is highly desirable.
• Knowledge of electronic health record systems to facilitate efficient documentation and patient tracking.
• Engagement in continued medical education (CME) to stay abreast of the latest advancements in Cardiology and nursing best practices.

Compensation and Benefits

The compensation for this role is competitive, ranging from $120,000 to $160,000 annually, commensurate with experience and qualifications. Benefits include:
