Key Responsibilities

• Conduct comprehensive evaluations, including reviewing patient history, performing specific assessments, and determining appropriate treatment recommendations while meticulously documenting results.
• Communicate evaluation findings and treatment plans to patients and their families, empowering them to participate in goal-setting and rehabilitation strategies.
• Implement personalized treatment protocols based on individual evaluations and adhere to the latest healthcare standards.
• Consult and collaborate with fellow healthcare professionals to ensure a comprehensive approach to patient care.
• Monitor and document changes in patients' conditions, adjusting plans of care as necessary to achieve optimal recovery outcomes.
• Manage discharge planning and follow-up care effectively, ensuring continuity of care through proactive communication and support.
• Document all patient care activities punctually and accurately, maintaining clear and detailed records encompassing evaluations, plans of care, progress notes, and discharge evaluations.
• Exhibit excellent time management skills while meeting productivity expectations and maintaining a positive attitude towards colleagues and patients.
• Participate in marketing initiatives that promote community awareness and understanding of our services and treatment approaches.
• Contribute to departmental objectives by actively engaging in quality improvement initiatives and educational endeavors.
• Maintain compliance with all relevant governmental and accreditation regulations.
• Pursue continuing education opportunities to enhance personal and organizational competence.
• Serve as a clinical instructor, fostering the growth of students in their physical therapy training.
• Support initiatives aimed at removing barriers to access and improving patient care through data-driven insights.

Patient Population / Client Focus

Qualifications
