Key Responsibilities\n

• Conduct thorough evaluations and assessments of children to identify developmental delays or disabilities.

• Participate in the development and implementation of Individualized Family Service Plans (IFSP) tailored to the unique needs of each child and their family.
\n
• Monitor and assess outcomes to ensure children meet developmental milestones, adjusting plans as necessary.
\n
• Provide hands-on support to families through education and resource referral, fostering a nurturing and supportive environment for their child's growth.
\n
• Utilize evidence-based practices to collaboratively create strategies and recommendations with families, focusing on natural learning environments.
\n
• Ensure compliance with all Federal and State regulations governing pediatric therapy services.
\n
• Build strong rapport with children and families to instill trust and encourage open communication about their needs and goals.
\n
• Employ coaching models to enhance parent and caregiver skills in areas related to child development.

Patient Population / Client Focus\n

The primary focus of this position is on young children, from infants to toddlers, who may be experiencing developmental delays or disabilities. You will work closely with families, utilizing a family-centered approach to care which prioritizes the child's best interests while considering each family's unique circumstances and cultural background. By actively engaging in their child’s early education and development, you contribute substantially to improving children's lives.

Qualifications\n

• A Master's Degree or Doctorate in Social Work from an accredited institution.

• Active LCSW license issued by the Department of Public Health.

• Valid driver's license required for travel to client homes and community settings.
