Key Responsibilities

As a Physical Therapist at Vitality Restore PT, your day-to-day tasks will include:

• Conducting comprehensive patient assessments to establish individual rehabilitation needs and contribute to the development of personalized care plans.
• Implementing skilled therapeutic interventions and exercises within the home, aligning with each patient's physician-directed plan of care.
• Monitoring patient progress and making necessary adjustments to treatment plans based on observed improvements or challenges.
• Educating patients and their families about therapy techniques, home exercises, and the effective use of necessary therapeutic devices.
• Maintaining accurate documentation of patient evaluations, treatment plans, progress notes, and outcomes in compliance with industry standards.
• Collaborating with other healthcare professionals to ensure holistic care and effective communication regarding patient status.
• Managing your caseload with efficiency, prioritizing tasks and coordinating care based on individual patient needs.
