Key Responsibilities

As a Physical Therapist at Upstream Physical Medicine, your daily responsibilities will include:

• Conducting comprehensive evaluations and assessments of patients referred by physicians, ensuring accurate diagnosis and treatment planning.
• Developing and implementing personalized treatment plans that utilize evidence-based protocols to promote recovery and rehabilitation.
• Administering therapeutic interventions tailored to diverse patient needs, including manual therapy, therapeutic exercise, and modalities to support healing.
• Offering education, guidance, and training to patients and their caregivers on exercises, injury prevention, and wellness strategies.
• Documenting patient progress and modifying treatment plans accordingly to meet rehabilitation goals.
• Engaging with interdisciplinary teams to enhance patient outcomes, sharing insights and collaborating on treatment approaches.
• Meeting operational goals and performance targets to ensure the clinic's efficiency and effectiveness.

Patient Population / Client Focus

The patients you will serve encompass a wide range of demographics, including orthopedic, pediatric, neurologic, and sports medicine populations. Each day, you will work with individuals on their unique healing paths, from young athletes recovering from sports injuries to seniors navigating chronic pain. Your role is crucial to not only restoring physical function but also fostering emotional support and confidence in your patients throughout their recovery processes.

Qualifications

To be considered for this role, candidates must have:

• A Bachelor's degree from an APTA or CAPTE-accredited physical therapy program; a Doctorate in Physical Therapy is preferred.
• A valid State License in Physical Therapy to practice in New York.
• A Basic Life Support (BLS) Certification from the American Heart Association.
• Strong written and verbal communication skills to foster effective interaction with patients and team members.
• Proficiency in word processing and documentation software.
• A collaborative spirit with a commitment to building meaningful relationships with patients and colleagues alike.

Preferred Experience

While not mandatory, the following experiences will enhance your candidacy:

• Previous clinical experience in a rehabilitation or outpatient setting.

Compensation and Benefits
