Work Environment

As a Physical Therapist in our home care setting, you will be visiting patients in their residences, providing therapy tailored to their specific needs. Key Responsibilities

Your role will encompass a variety of responsibilities designed to ensure effective patient care:

• Develop personalized treatment plans based on thorough evaluations of patients' capabilities, impairments, and goals.
• Administer manual therapy techniques, therapeutic exercises, and activities to enhance mobility, strength, and functionality.
• Implement and instruct patients in daily exercises that assist their recovery, while providing motivation and support throughout their rehabilitation journey.
• Utilize diagnostic assessments to evaluate muscle, nerve, and joint function, and modify treatment plans as needed.
• Fit and adjust orthotic and prosthetic devices, working closely with orthopedic specialists to ensure optimal patient outcomes.
• Maintain accurate records of patient progress using electronic medical record systems to enhance communication and continuity of care.

Patient Population / Client Focus

Your primary focus will be on patients recovering from orthopedic injuries or surgeries, including fractures, joint replacements, and sports-related injuries. You will work with a diverse clientele, including elderly individuals, active adults, and those needing post-operative rehabilitation. Your expertise will guide patients through evidence-based interventions, aimed at restoring independence and enhancing their quality of life.

Qualifications

• A Master's Degree or Doctorate in Physical Therapy from an accredited program.
• Active State license as a Physical Therapist.
• At least two years of experience in orthopedic settings, demonstrating proficiency in assessment and treatment.
• Exceptional communication skills, with the ability to interact empathetically with patients and families.
• Organizational skills that enable you to manage multiple patient care plans efficiently within a busy home care schedule.
• Familiarity with electronic medical record systems and documentation practices.

Preferred Experience

While we require a minimum of two years of experience in physical therapy, preference will be given to candidates with specialized training in orthopedic rehabilitation, geriatric care, or outpatient physical therapy services. Additionally, proficiency with advanced therapy techniques such as dry needling or manual therapy will be considered a strong asset.

Compensation and Benefits
