Key Responsibilities\n

The Licensed Clinical Social Worker will:

  Assess, diagnose, and evaluate patients using evidence-based tools, including the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), to devise individualized care plans that consider cultural diversity and unique patient needs.

  Regularly monitor patient progress, adjusting therapeutic strategies as necessary to effectively address mental health diagnoses and facilitate positive outcomes.

  Prepare and maintain accurate and timely records of treatment, adhering to regulatory requirements and departmental standards while utilizing current electronic health record (EHR) systems.

  Collaborate with patients and their families to construct comprehensive treatment plans focused on mutually agreed-upon goals, ensuring the inclusion of necessary interventions and support mechanisms.

Patient Population / Client Focus\n

You will work with a diverse array of clients in an outpatient setting, ranging from children and adolescents to adults dealing with a variety of mental health conditions.
