Key Responsibilities\n

• Deliver therapeutic services that aid patients in managing mental health challenges associated with acute, chronic, and terminal conditions.

• Conduct thorough assessments to determine individual patient needs and formulate personalized care plans, collaborating closely with healthcare teams.

• Provide advocacy for patients and their families, actively promoting patient rights and ensuring access to necessary support services.

• Utilize electronic health records (EHR) for documentation, monitoring progress, and communicating effectively with the interdisciplinary team.

• Engage in care coordination efforts to prevent hospital readmissions, including identifying risk factors and participating in transition planning conferences.

• Encourage and facilitate family involvement in care processes, offering education and emotional support.

• Stay abreast of regulatory requirements regarding potential patient vulnerabilities and reporting obligations.

Qualifications\n

Compensation and Benefits\n
