Key Responsibilities

As a Licensed Clinical Professional Counselor at Crystal Lake Families & Kids Services, your responsibilities will encompass a variety of patient-focused actions:

• Conduct initial patient intakes, utilizing diagnostic and functional evaluations to inform treatment planning.
• Provide brief, focused mental health interventions tailored to the needs of each client.
• Collaborate with your team and primary care providers to assess clients' needs and develop comprehensive treatment plans.
• Monitor client progress, maintain clinical records, and document all relevant information in treatment charts to comply with Agency and Federally Qualified Health Center guidelines.
• Develop and implement relapse prevention strategies for patients at risk of deterioration.
• Assist in the identification of at-risk individuals and create plans to prevent the worsening of their conditions.
• Facilitate efficient communication with primary care providers regarding patient needs, progress, and necessary referrals to specialized mental health services.
• Provide education and resources aimed at promoting self-management and healthier lifestyle choices for clients and their families.

You will engage with a diverse patient population, primarily consisting of families and children. Qualifications

To qualify for this role, candidates must possess the following:

• A minimum of a Master’s Degree in Clinical Counseling.
• A current Licensed Clinical Professional Counselor (LCPC) credential.
• Deep knowledge of mental health assessment, treatment planning, and behavioral health methodologies.
• Familiarity with DSM-IV diagnoses and psychotherapeutic techniques.
• Capability to establish positive working relationships with patients, families, and multidisciplinary teams.
• Ability to maintain clinical records meticulously and articulate assessments clearly.
• Sound judgment and decision-making skills based on professional standards.

Preferred Experience

While not required, bilingual proficiency in English and Spanish is preferred, enabling better communication with our diverse clientele. Experience working in outpatient settings and familiarity with telehealth practices would be advantageous, enhancing your ability to reach and assist patients effectively.

Compensation and Benefits

The position offers a competitive salary ranging from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
