Key Responsibilities\n

• Coordinate care by consulting with physicians regarding patient progress, needs, concerns, and any potential or actual problems that arise.

• Maintain accurate and timely clinical records, including progress notes and treatment plans, in compliance with departmental policies and regulatory requirements.

• Develop comprehensive treatment and discharge plans based on psychosocial assessments and accepted clinical interventions, collaborating closely with other healthcare professionals.

• Assist clients with discharge planning, managing care coordination among various service providers in the region to facilitate smooth transitions and access to necessary resources.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Marriage and Family Therapy, Psychology, or a closely related field.
