This full-time position will primarily focus on providing high-quality physical therapy services in home care settings, with a competitive salary range of USD 100,000 to 120,000 annually. Key Responsibilities

• Conduct thorough evaluations of patients by reviewing pertinent medical histories, performing detailed assessments, and documenting results to formulate individualized treatment plans.
• Communicate evaluation findings and proposed treatment plans clearly to patients and their families, actively involving them in setting achievable goals for rehabilitation.
• Implement evidence-based treatment protocols tailored to each patient's condition, adapting to their progress and changes in health status.
• Collaborate with a multidisciplinary team of healthcare providers to ensure comprehensive care, documenting any changes in patient condition to modify the treatment plan as needed.
• Oversee discharge planning and follow-up care to ensure continued progress post-treatment, maintaining meticulous records of patient interactions and progress.
• Manage your caseload effectively to meet designated productivity expectations, while also participating in community-based marketing efforts to promote accessibility to care.
• Contribute to departmental goals, maintaining compliance with regulatory standards and accreditation requirements.
• Engage in ongoing professional development by pursuing continuing education opportunities and acting as a mentor for interns and new staff.
• Enhance the patient experience by being adaptable and responsive to their needs, effectively addressing customer service queries.

Patient Population / Client Focus

Your primary focus will be on patients navigating orthopedic challenges, including post-surgical recovery, injury rehabilitation, and chronic pain management. You will work with a diverse range of individuals, adapting your approach to meet the unique needs of each patient. This role emphasizes building strong therapeutic relationships that empower patients to achieve their rehabilitation goals and improve their quality of life.

Qualifications

• A Doctorate of Physical Therapy (DPT) degree from an accredited institution.
• Current State Licensure as a Doctor of Physical Therapist (DPT) in Virginia.
• A minimum of two years of professional experience in a clinical setting, preferably in orthopedic rehabilitation.
• Current CPR certification to ensure the safety and well-being of all patients.

Preferred Experience

While a minimum of two years of experience is required, candidates with experience in home health settings, knowledge of electronic health record systems, or those with specialized certifications will be given preference.
