Work Environment

This position requires on-site work in home care settings, allowing for a unique and intimate approach to patient treatment. You will travel to patients' residences, allowing you to provide personalized therapy that addresses their specific needs. Key Responsibilities

• Conduct comprehensive evaluations of patients in their residences to assess their physical capabilities and challenges as they recover from injuries, illnesses, or surgeries.
• Develop individualized treatment plans that utilize a variety of therapeutic methods, including exercise programs, manual therapy, and the use of assistive devices to minimize pain and enhance mobility.
• Implement therapy sessions in a way that aligns with the established treatment goals, adapting techniques and providing encouragement to optimize patient engagement and outcomes.
• Document progress meticulously, tracking each patient's response to therapy and revising treatment plans as necessary to ensure continual improvement.
• Educate patients and their families on the use of assistive devices and self-care strategies to foster independence and confidence in managing their physical health.
• Collaborate with other healthcare professionals, including physicians and occupational therapists, to ensure a holistic approach to patient care.
• Continuously maintain and upgrade professional knowledge through workshops, seminars, and participation in relevant professional organizations.

Patient Population / Client Focus

The Home Care Physical Therapist at Amethyst will work with a diverse range of patients, from children requiring rehabilitation after an injury to elderly adults recovering from surgery or chronic conditions. This includes designing adaptive treatments that cater specifically to each client's unique capabilities and lifestyles, thereby ensuring they achieve the best possible outcomes in their recovery journeys.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• Current and valid physical therapy license in the State of New Jersey.
• Exceptional verbal and written communication skills to effectively interact with patients, families, and a multi-disciplinary team.
• Proficient in using digital documentation tools and maintaining organized patient records.
• A flexible and proactive approach toward patient care and collaboration.

Preferred Experience

While a minimum of two years of experience is preferred, we welcome applicants who may not meet this requirement but demonstrate strong foundational skills and eagerness to learn. Mentorship for new graduates is available, helping you gain confidence and competence in your practice.

Compensation and Benefits
