Physical Therapist - Full Time - Home Health Care

Position Overview Trigger Physical Therapy is seeking a full-time Physical Therapist to enhance our home care services in Toms River, New Jersey. Our home care facility allows therapists to deliver personalized attention and care directly in patients' comfort zones, ensuring a conducive atmosphere for recovery. The work hours are Monday through Friday, from 9 AM to 5 PM, allowing a structured schedule that supports work-life balance.

Conduct thorough assessments to identify and address individual patient needs, ensuring an optimal approach tailored to each healthcare scenario.

Manage a caseload efficiently, performing regular sessions that ideally encompass one patient per hour, thus maintaining quality and focused attention.

Utilize manual therapy techniques and evidence-based practices to improve patients' conditions, monitoring progress closely and adjusting therapies as warranted.

Maintain accurate and timely documentation covering patient evaluations, treatment plans, and progress notes to adhere to professional and regulatory standards.

Collaborate with a multidisciplinary team of healthcare professionals to coordinate care and optimize patient results, fostering a holistic approach to health and wellness.

This role primarily involves working with a diverse array of patients in home care settings, encompassing various age groups and physical conditions. From post-operative rehabilitation to chronic pain management, the Physical Therapist will engage with clients whose circumstances require personalized and adaptive therapeutic strategies, all while prioritizing patient comfort and well-being.

A Doctorate in Physical Therapy (DPT) from an accredited institution.

A valid New Jersey physical therapy license, with a record free from disciplinary actions or board sanctions.

Minimum of 2 years of relevant experience providing physical therapy services.

Demonstrated clinical mastery in manual therapy techniques and outpatient orthopedic practices.
