Conduct thorough assessments to examine, evaluate, and develop personalized treatment plans for patients experiencing functional limitations and disabilities.

Maintain open lines of communication with interdisciplinary team members, including referral sources and physicians, to facilitate collaboration and enhance treatment efficacy.

This role primarily serves a diverse population of patients requiring rehabilitative care in the comfort of their homes. Understanding cultural sensitivities and patient individuality is critical to providing effective therapy.

Completed a Bachelor's Degree in Physical Therapy from an accredited institution.

Possession of a valid State License in Physical Therapy and Basic Life Support (BLS) certification.

familiarity with electronic health records and essential computer skills is advantageous.

A minimum of two years of experience in a clinical setting, especially in home care or outpatient therapy environments, is preferred. Experience working with diverse patient demographics is a significant asset.
