Key Responsibilities

• Conduct thorough evaluations of patients by reviewing medical history, implementing specific assessments, and documenting findings, leading to tailored treatment plans.

• Develop and execute treatment protocols based on individual patient evaluations, taking into consideration their specific health presentations and the evolving landscape of healthcare.
• Collaboratively consult with healthcare professionals as needed, maintaining comprehensive documentation of any changes in patient conditions and adjusting care plans accordingly.
• Implement effective discharge planning and follow-up care, ensuring accurate and timely documentation that reflects the patient's journey, progress, and overall treatment efficacy.

• Facilitate the achievement of departmental objectives while ensuring compliance with accreditation and regulatory standards.

Patient Population / Client Focus

As a Physical Therapist within our home care setting, you will work with a diverse patient population, including seniors recovering from surgery, individuals managing chronic pain, and patients requiring rehabilitation after significant health events. Your role will be pivotal in addressing the unique challenges these patients face in a home environment, ensuring that you promote their autonomy while providing expert care tailored to their specific needs.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required.
• A high school diploma or GED is also required.
• Licensure to practice as a Physical Therapist in the state of Virginia is essential.
• Certification in Basic Life Support (BLS) for Healthcare Providers, as recognized by the American Heart Association.

Compensation and Benefits
