Key Responsibilities\n

• Conduct comprehensive assessments of clients, ensuring that their individual needs and circumstances are understood and addressed.

• Collaborate effectively with supervisors, healthcare professionals, families, and guardians to discuss client progress and tailor treatment plans accordingly.

• Develop and maintain a clear schedule of appointments, home visits, and monitoring observations to track and evaluate client outcomes.

• Carry out and coordinate essential in-home visits, including chart reviews and interviews, to gain deeper insights into each client's situation.

• Oversee the implementation of personalized program plans, ensuring they are executed effectively and align with best practices.
\n
• Engage actively in Interdisciplinary Team Meetings to foster collaboration and share updates regarding client care.
\n
• Document and complete monthly and quarterly progress notes, maintaining meticulous records that are accessible to stakeholders.
\n
• Organize and provide training sessions for staff and team members to ensure adherence to program plans and documentation protocols.
\n
• Conduct follow-up assessments and home visits to offer ongoing support and facilitate continued progress for clients.

• Represent Congruent Health Center in communication with the public and other entities in the mental health community to foster developmental goals.

Patient Population / Client Focus\n

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related mental health field is essential.

• Current active licensure as a Professional Counselor (LPC) in the state of California without restrictions.

• A minimum of 2 years of experience in a clinical counseling setting.

• Strong foundational knowledge of legal and ethical standards related to mental health practice.
\n
• Exceptional interpersonal and communication skills, adept at establishing rapport with diverse client demographics.

• Demonstrated ability to collaborate effectively with a multifaceted healthcare team.

• Familiarity with electronic medical record (EMR) software and technology relevant to mental health services.
