Work Environment\n

This role is primarily remote, allowing for a comfortable work environment while maintaining a high level of interaction with patients. You will conduct sessions through telehealth platforms or phone calls, enabling flexibility in scheduling and accessibility for our clients. Key Responsibilities\n

• Collaborate closely with primary care providers to identify, treat, and manage mental health and psychosocial issues, serving as a vital member of the interdisciplinary healthcare team.

• Conduct thorough patient intakes, emphasizing diagnostic evaluations and functional assessments, followed by developing tailored treatment plans in conjunction with primary care providers.

• Implement brief, focused interventions for patients requiring mental health services, employing evidence-based therapeutic techniques.
\n
• Make informed recommendations regarding the referral of patients to either primary care or specialized mental health facilities based on their individual needs.
\n
• Follow up with patients to monitor progress, discuss treatment efficacy, and adjust approaches as necessary, fostering an ongoing therapeutic relationship.

• Design and help implement relapse prevention plans to support patients in maintaining stable functioning and reducing the risk of setbacks.
\n
• Identify at-risk patients and collaboratively develop actionable plans to preemptively address worsening conditions.
\n
• Document patient progress accurately in treatment charts and maintain clear communication with primary care providers regarding patient needs and treatment developments.

• Educate patients on self-management skills and wellness strategies to empower their active participation in their recovery journeys.
\n
• Consult with clinic management and team members on behavioral services and necessary program evaluations to enhance service delivery.

• Adhere strictly to the Code of Ethical Conduct for Clinical Professionals and perform additional related duties as assigned.

Patient Population / Client Focus\n

Our LPCs will work with a diverse patient population, including individuals facing a myriad of challenges such as anxiety, depression, trauma, and other mental health issues. We prioritize a holistic approach, considering the impact of social determinants of health on our patients' well-being. You will be working to create a supportive environment that facilitates growth and healing, employing both individual and group therapy methodologies.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a related field.

• Active and current LPC license in Pennsylvania is required.

• Proficient understanding of mental health assessments, treatment planning, psychotherapeutic techniques, and diagnoses outlined in the DSM-IV.

• Strong communication and interpersonal skills to facilitate rapport with patients and healthcare colleagues.

• Ability to handle sensitive information with the utmost confidentiality and ethical considerations.

Preferred Experience\n

• Bilingual proficiency in English and Spanish is highly desirable but not mandatory.

• Previous experience in a similar outpatient setting, particularly with integrated care approaches, is advantageous.

• Familiarity with community resources and their utilization in support of patient care is preferred.

Compensation and Benefits\n

This full-time position offers a competitive yearly salary ranging from USD 87,000 to 105,000, based on experience and qualifications. Additionally, we provide a SIGN-ON BONUS to newly hired counselors.
