Transformative Psychotherapy Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our innovative hospice center in Corpus Christi, Texas. This is a full-time remote position where you will provide essential mental health services tailored specifically for patients in hospice care, helping them and their families navigate emotional and psychological challenges during some of life’s most difficult transitions.

Key Responsibilities\n

• Conduct thorough psychological assessments for patients during their intake process, creating comprehensive treatment plans that address their unique mental health needs.

• Deliver individual, group, and family counseling sessions using evidence-based therapeutic techniques, addressing issues such as grief, anxiety, and depression related to end-of-life experiences.

• Monitor and evaluate client progress, adjusting treatment plans as necessary to ensure effective care and additional support when needed.

• Collaborate within a multidisciplinary team, engaging with physicians, nurses, and chaplains to coordinate holistic care and address complex familial dynamics.

• Implement crisis intervention strategies for clients experiencing acute distress, providing immediate support and connecting them with appropriate resources.
\n
• Maintain accurate and timely documentation of client interactions, care plans, and outcomes in adherence to HIPAA regulations and organizational standards.

• Stay current with developments in social work and hospice care through continuous professional education and participation in relevant training.

Patient Population / Client Focus\n

Your primary focus will be on patients facing terminal illnesses, as well as their families, who often experience a vast array of emotions, including fear, sadness, and uncertainty. You will develop therapeutic relationships with clients while providing them with the tools to cope and find solace during challenging times. Our goal is to enhance the quality of life for both patients and their families by supporting their mental, emotional, and spiritual needs.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current, valid license to practice clinical social work in Texas.
\n
• Strong interpersonal and communication skills, promoting rapport with clients and teamwork within the multidisciplinary healthcare setting.

• Capability to manage sensitive information with confidentiality and professionalism.
\n
• Proficient in using teletherapy platforms and maintaining effective communication through technological means.

Preferred Experience\n

• Experience working in hospice care or with patients facing end-of-life issues is highly preferred.

• Familiarity with case management and community resource utilization to provide comprehensive support to clients.

• Prior experience in crisis intervention and grief counseling would also be advantageous.
