Key Responsibilities\n

• Provide compassionate mental health counseling, education, and support for patients facing various psychological challenges, along with their families.

• Ensure compliance with mandated reporting of suspected abuse or vulnerability in accordance with legal and ethical standards.

• Engage in multidisciplinary care coordination to avert unnecessary hospital readmissions, proactively addressing patients' needs.

• Lead transition conferences with patients and their families, facilitating a smooth transition planning process.

• Utilize electronic medical record systems to document treatments, monitor patient progress, and communicate effectively with the healthcare team.

• Collaborate with interdisciplinary teams, anticipating needs and moving patient-centered care plans forward.

• Assist patients and families with financial and social resource navigation, ensuring they are connected to necessary support services.

• Advocate for patients’ rights and collaborate with community resources to reinforce care accessibility and support.

• Foster a therapeutic environment that promotes patient rights, ethical decision-making, and proactive engagement.

• Maintain comprehensive knowledge of the local healthcare landscape, including government and private payer networks.

Qualifications\n
