Key Responsibilities

• Conduct comprehensive evaluations of patients in a home care setup to identify their unique rehabilitation needs and goals.
• Develop and implement individualized treatment plans that integrate various therapeutic modalities such as manual therapy techniques, therapeutic exercises, and modern rehabilitation tools to facilitate sustained recovery.
• Utilize standardized assessment protocols to track patient progress, adjusting treatment strategies in response to clinical findings.
• Educate patients and their families on therapeutic exercises and self-care strategies, providing training on assistive devices to enhance mobility and independence.
• Maintain accurate and detailed documentation of patient evaluations, treatment plans, and progress notes to inform future care and comply with healthcare regulations.
• Participate in regular training and professional development opportunities to enhance clinical skills and stay informed on the latest therapy techniques and equipment.

Patient Population / Client Focus

Our practice serves a diverse patient population, from pediatric to geriatric individuals facing mobility issues due to injury, illness, or post-surgical recovery. Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid physical therapy license to practice in the state of Florida.

• Proficient in utilizing electronic health record (EHR) systems and other computer-based tools for documenting patient care.
• Highly adaptable, with a proactive approach to patient treatment and self-management.

Preferred Experience

• A minimum of 2 years of clinical experience in physical therapy, preferably in a rehabilitation setting.
• Experience with home care therapy models is advantageous but not mandatory.

Compensation and Benefits
