Key Responsibilities\n

• Conduct comprehensive and ongoing assessments to identify each patient's unique needs, ensuring that appropriate interventions are personalized and effective.

• Maintain open lines of communication among supervisors, direct care staff, family members, and other involved parties, ensuring everyone is informed about each client's progress and treatment plans.

• Develop and share a planned schedule with supervisors for appointments, home visits, and other necessary observations to monitor client progress.

• Coordinate essential home visits for in-home observations, chart reviews, and collaboration with support staff, ensuring that all findings are accurately documented and addressed.

• Implement and evaluate the effectiveness of individualized program plans, ensuring they meet the client's needs and compliance with best practice standards.
\n
• Actively participate in Interdisciplinary Team Meetings, contributing valuable insights and recommendations regarding treatment approaches.
\n
• Document monthly and quarterly progress notes, providing detailed updates that reflect each client’s evolving treatment journey.
\n
• Organize and conduct training sessions to ensure that all staff members understand and effectively implement program plans and documentation processes.
\n
• Engage in follow-up interviews and home visits to assess ongoing support requirements and to help clients navigate their treatment pathways.
\n
• Maintain comprehensive and organized client records, facilitating easy access to information for authorized stakeholders.
\n
• Serve as a liaison within the mental health community, fostering productive relationships to support community engagement and development initiatives.

• Assist in the creation of training materials aimed at enhancing the quality of programs offered to individuals.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is mandatory.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in Maryland is required.

• A minimum of two years of counseling experience, preferably within a clinical context, is essential for this role.

• A strong understanding of ethical and legal considerations in mental health practice is essential.

Compensation and Benefits\n
