Key Responsibilities

• Deliver high-quality, compassionate care to patients in both acute and outpatient settings, ensuring safety and effectiveness.

• Implement individualized treatment programs for assigned patients based on comprehensive treatment plans developed in collaboration with other healthcare professionals.

• Conduct thorough evaluations, gathering relevant medical history, performing specialized assessments, and documenting findings to inform treatment decisions.

• Engage in comprehensive discharge planning by formulating follow-up care strategies, maintaining accurate documentation throughout the patient's rehabilitation journey to substantiate care efficacy.

• Ensure compliance with all corporate, clinical, and regulatory requirements by completing mandatory training and remaining up-to-date with local, state, and federal regulations.

• Evaluate and determine the medical necessity for physical therapy, clearly documenting the rationale for treatment modalities.

• Foster respectful and open communication with colleagues, physicians, patients, and families, ensuring cohesive care across multidisciplinary teams.

• Participate in team meetings and professional development sessions as directed by management to enhance care practices and patient outcomes.

Patient Population / Client Focus

Our center primarily serves patients with orthopedic conditions requiring rehabilitation. You will engage with a diverse population ranging from post-operative recovery cases to individuals with chronic pain. Qualifications

• Possession of a Doctor of Physical Therapy (DPT) degree from an accredited institution.

• A current state licensure as a Physical Therapist is required.

• American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers must be obtained prior to starting the role.

• Strong foundational knowledge of orthopedic conditions and treatment protocols is essential.

Preferred Experience

• A minimum of 2 years of experience in an orthopedic physical therapy setting is highly preferred. Experience in home health care services is a plus.

• Familiarity with electronic health records (EHR) systems and telehealth platforms to enhance patient interactions and documentation processes.
