Key Responsibilities\n

• Conduct comprehensive and ongoing assessments to evaluate the needs of patients, ensuring each individual receives tailored therapeutic interventions.

• Communicate effectively with supervisors, direct care team members, and family stakeholders regarding the progress of clients and necessary updates on treatment plans.

• Design and share a tentative schedule with supervisors outlining appointments, home visits, and observations to track progress and manage caseload effectively.

• Coordinate and conduct home visits for comprehensive assessments, including chart reviews and staff interviews, to offer a holistic view of the patient’s environment and support system.

• Ensure effective implementation of established program plans, continually assessing their effectiveness and adapting when necessary to optimize client outcomes.

• Participate in interdisciplinary team meetings and additional relevant gatherings to discuss case complexities and collaborate on client-centric strategies.

• Compile and document progress notes monthly and quarterly to ensure compliance and facilitate ongoing evaluation of treatment efficacy.

• Organize and lead training sessions aimed at enhancing team competency regarding the implementation of program plans and documentation processes.

• Provide continuous follow-up services through interviews and observations, ensuring that clients’ evolving needs are met with ongoing support.

• Maintain meticulous records for all clients while guaranteeing that pertinent information is accessible to authorized parties to support effective treatment planning.

• Act as a liaison with the wider mental health community, aiding in the development of support structures and resource allocation for clients.

• Assist in drafting procedures for training materials aimed at improving program methodologies and documentation accuracy.
