Work Environment

This position is primarily onsite, delivering care directly in the patient's home. Key Responsibilities

As a Home Care Physical Therapist, your day-to-day responsibilities will include:

• Conducting thorough evaluations to assess the functional abilities of patients referred by physicians and tailor treatment plans accordingly.
• Implementing evidence-based therapeutic techniques and interventions aimed at improving mobility, strength, and overall function.
• Working collaboratively with multidisciplinary teams to maximize patient outcomes, sharing insights and support in treatment approaches.
• Providing education and consultation to patients and their caregivers, ensuring they understand the treatment process and how to contribute to their recovery.
• Meeting minimum utilization goals while maintaining the highest standards of clinical practice and customer service.
• Documenting patient progress and adjusting treatment plans based on ongoing assessments and feedback.

Patient Population / Client Focus

You will primarily work with a diverse group of patients, including those with orthopedic, neurological, pediatric, and sports-related conditions. Your role is vital in helping these individuals regain their functionality and independence, guiding them through tailored and purposeful therapeutic interventions in a familiar setting—their own home.

Qualifications

To succeed in this role, candidates must meet the following qualifications:

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid DPT license in the State of Virginia.
• Graduation from an APTA or CAPTE-approved physical therapy program.
• Basic Life Support (BLS) Certification from the American Heart Association.
• Strong written and verbal communication skills, with proficiency in word processing and computer applications.

Preferred Experience

While a minimum of two years of experience as a physical therapist is required, candidates with additional experience in home healthcare, diverse patient populations, or specialization in specific therapeutic areas will be highly regarded. Compensation and Benefits
