Key Responsibilities

As a Physical Therapist at Upstream, your daily responsibilities will include:

• Evaluating patients' physical abilities and needs through comprehensive assessments.
• Developing individualized treatment plans in collaboration with physicians and healthcare team members.
• Implementing a range of therapeutic interventions, including manual therapy, modalities, and exercise programs tailored to each patient's condition.
• Monitoring patient progress and modifying treatment plans as necessary to achieve optimal outcomes.
• Providing guidance and education to patients and their caregivers regarding home exercise programs and self-management strategies.
• Maintaining accurate documentation of patient treatments, outcomes, and overall progress in compliance with organizational standards.
• Contributing to team meetings and case discussions to improve treatment approaches and patient outcomes.

Patient Population / Client Focus

Our client base includes individuals across various demographics, with conditions ranging from orthopedic injuries to neurological impairments. Our physical therapist will work with diverse specialties, including:

• Orthopedic cases such as post-surgery rehabilitation and chronic pain management.
• Pediatric patients requiring physical therapy for developmental delays or injuries.

Qualifications

The ideal candidate will possess the following qualifications:

• A Bachelor's degree from an accredited physical therapy program, with a Doctorate in Physical Therapy preferred.
• Current state licensure in Physical Therapy.
• Basic Life Support (BLS) certification from the American Heart Association.
• Strong written and verbal communication skills.
