Key Responsibilities\n

In this role, you will:

• Conduct thorough patient intakes, focusing on diagnostic evaluations and functional assessments to inform individualized treatment goals and plans.

• Provide brief and targeted therapeutic interventions for patients experiencing mental health challenges, ensuring adherence to established Agency and Federally Qualified Health Center (FQHC) guidelines.

• Collaborate closely with primary care providers to identify patients' needs, recommend treatment plans, and monitor ongoing mental health progress.

• Develop and implement relapse prevention strategies for at-risk patients to promote stability and ensure continuous support for their recovery journey.

• Document patient assessments, intervention plans, and progress in compliance with clinical standards and maintain detailed treatment charts.

• Engage in follow-up communication with patients to evaluate treatment effectiveness and make necessary adjustments to care plans.

• Assist in educating patients about self-management techniques and strategies to cope with medication side effects or psychosocial barriers.

• Support continuous learning and treatment improvement by providing feedback and consultation on behavioral health practices to clinic management.

The ideal candidate must possess:
