Work Environment

This role is primarily onsite, providing therapy services directly in clients' homes. With a typical work schedule from Monday to Friday, 9 AM to 5 PM, our therapists enjoy the balance of professional commitment and personal time, contributing to overall job satisfaction and effectiveness in patient care.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities, integrating assessment tools and techniques to develop personalized treatment plans.
• Deliver skilled therapy services, including therapeutic exercises, manual therapy, and various treatment modalities tailored to meet each patient's unique needs.
• Regularly monitor and document patients' progress, utilizing electronic medical records (EMRs) for accurate tracking and to support evidence-based practice.
• Collaborate with a multidisciplinary team of healthcare professionals, including physicians and occupational therapists, to establish coordinated care plans.
• Educate patients and their families about therapeutic exercises, injury prevention strategies, and the importance of adherence to treatment regimens.
• Implement and advocate for fall prevention programs and safety measures, ensuring a safe environment within the assisted living community.
• Engage in continuing education opportunities to stay current with innovative practices and enhance your clinical skills, ensuring the best care for our clients.
• Uphold ethical and professional standards, maintaining confidentiality and commitment to delivering optimal patient care.

Patient Population / Client Focus

Your primary focus will be on the geriatric population, addressing their specific physical therapy needs related to chronic illnesses, mobility challenges, and rehabilitation after surgeries. You will empower elderly clients by providing compassionate care and effective treatment plans designed to foster independence and enhance their quality of life.

Qualifications

• A graduate with a Bachelor's degree or higher from an accredited institution in Physical Therapy.
• Current and active State license to practice as a Physical Therapist.
• Strong interpersonal and communication skills, enabling effective interaction with clients, their families, and other healthcare professionals.
• Proficiency in electronic medical records (EMRs) systems.
• Knowledge of evidence-based practices and the ability to apply the latest therapeutic techniques tailored specifically for the geriatric population.

Preferred Experience

• Previous experience in providing geriatric therapy is highly desirable, though new graduates are encouraged to apply and will receive mentorship.
