As a Licensed Mental Health Counselor, your key responsibilities include:

• Ensuring mandated reporting of any vulnerabilities or suspected abuse occurs appropriately.

• Engaging in practices that reduce the likelihood of hospital readmissions through strategic care planning.

• Conducting and participating in transition conferences aimed at fostering client and family engagement in care pathways.

• Employing digital tools and technology to proactively identify and intervene for patients at risk of readmission.

• Facilitating comprehensive clinical handoffs for at-risk patients, including necessary referrals.

• Providing counseling and ongoing support focused on financial and social needs to ensure the well-being of patients and their families.

• Documenting patient progress and treatment plans through an electronic medical records system, ensuring effective tracking and communication of care outcomes.
