Work Environment

Our Home Care Physical Therapists operate in patients' homes throughout Fort Lauderdale, allowing for personalized assessment and treatment in a comfortable setting. Key Responsibilities

• Conduct comprehensive evaluations of patients' physical function and overall wellness, utilizing diagnostic tests to inform personalized treatment plans.
• Develop, implement, and adjust physical therapy treatment plans designed to prevent disability, relieve pain, and restore function based on individual capabilities.
• Direct and supervise Physical Therapy Assistants (PTAs) to ensure that therapy interventions align with established care plans, and provide guidance during treatment.
• Administer manual therapy techniques, functional exercises, and neuromuscular re-education to promote physical rehabilitation.
• Assess and recommend the use of assistive devices, such as prosthetics or orthotics, tailoring solutions for each client's mobility needs.
• Regularly document all treatment activities, progress notes, and modifications to care plans in compliance with clinical standards, including OASIS guidelines.
• Engage in interdisciplinary team meetings to discuss cases, share insights, and collaboratively adjust care approaches to achieve optimal outcomes.
• Instruct patients and their families on effective home exercise programs and the proper use and maintenance of therapy equipment and techniques.
• Maintain open lines of communication with physicians, case managers, and other healthcare professionals involved in the patient's care to facilitate coordinated treatment efforts.

Patient Population / Client Focus

This role primarily involves working with a diverse patient population, including elderly clients, patients recovering from surgeries, and individuals with chronic conditions affecting their mobility and strength. The Home Care Physical Therapist must be adept at customizing therapy strategies to meet the unique challenges and goals of each client, ensuring that care remains adaptive and responsive to their evolving needs.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution required.
• Active Physical Therapist license in Florida, free of sanctions or board proceedings.
• Exceptional knowledge and application of manual therapy modalities, exercise prescription, and rehabilitation techniques.
• Proficient in electronic medical record (EMR) systems, particularly Web PT, to ensure accurate documentation.

Preferred Experience

• Previous experience in home healthcare or outpatient rehabilitation settings is preferred.
• Strong ability to develop and adapt treatment protocols based on evolving patient needs and continuous feedback.
• Familiarity with telehealth services and patient education strategies is advantageous.

Compensation and Benefits

This full-time position offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience.
