Position Overview

Big Leap Family Guidance Centers is seeking dedicated and compassionate Licensed Professional Counselors (LPC) to join our innovative telehealth team. Work Environment

This position is designed for a remote work environment, predominantly utilizing telehealth platforms to provide counseling services. You will leverage technology to connect with patients, ensuring they receive the care they need from the comfort of their homes. Key Responsibilities

As a Licensed Professional Counselor at Big Leap Family Guidance Centers, you will:

• Assist primary care providers in recognizing, treating, and managing mental health and psychosocial challenges, becoming an integral part of the primary care team.
• Conduct comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations, leading to tailored treatment recommendations.
• Provide brief, targeted interventions for patients requiring mental health support, including crisis management and therapy sessions.
• Evaluate patients to determine if they should remain within the primary care setting or be referred to specialized mental health resources.
• Monitor patient progress, following up on treatment plans to ensure effectiveness and making adjustments as necessary.
• Develop relapse prevention plans and empower patients in maintaining stability in their emotional and mental health.
• Identify patients at risk of worsening conditions and create proactive strategies to mitigate these risks.
• Coordinate patient care with other providers while maintaining confidentiality and ethical practice standards.
• Document patient treatment plans, progress notes, and diagnostic information accurately and comprehensively.
• Educate patients on self-management techniques and healthier lifestyle choices, reinforcing their role in the treatment process.
• Engage in ongoing consultation with clinic leadership and team members regarding the quality and outcomes of behavioral health services.

Patient Population / Client Focus

Your work will primarily involve a diverse range of clients from various socioeconomic backgrounds, many of whom face complex mental health challenges. By fostering trusting relationships and providing culturally sensitive care, you will contribute significantly to improving their overall mental wellness and community integration.

Qualifications

To thrive in this role, candidates must possess the following qualifications:

• A minimum of a Master’s Degree in Clinical Counseling or a related field.
• A current Licensed Professional Counselor (LPC) license in the state of Connecticut.
• Comprehensive knowledge of mental health assessment, treatment planning, developmental psychopathology, and various therapeutic modalities.
• Strong understanding of psychodiagnostics, DSM-IV diagnoses, and behavioral therapies.
• The ability to work effectively in an interdisciplinary team setting and communicate deemed ideas clearly.
• A solid grasp of community resources available for patients and their impactful use.
• Strong organizational skills with an ability to maintain detailed clinical records and prepare relevant reports efficiently.

Preferred Experience

While not mandatory, the following qualifications are advantageous:

• Bi-lingual proficiency in English and Spanish, facilitating communication with a broader range of patients.
• Experience with telehealth or online counseling platforms, as well as familiarity with integrated care models.

Compensation and Benefits

This rewarding position offers a competitive salary range of USD 87,000 – 105,000 per year.
