Key Responsibilities

As a Physical Therapist specializing in home care, your primary responsibilities will include:

• Conducting initial evaluations to determine patients' physical limitations and specific rehabilitation needs post-injury, disease, or surgery.
• Designing and implementing individualized treatment plans incorporating a variety of techniques, such as therapeutic exercises, stretching, hands-on therapy, and specialized equipment.
• Tracking and documenting patient treatments, responses, and progress to ensure effective and continual care.
• Regularly reevaluating patient progress and adjusting treatment plans and goals as necessary based on observed outcomes and feedback.
• Providing training and education to patients and their families, as well as collaborating with other health professionals, to optimize the rehabilitation process.
• Staying updated with advancements in the field by participating in workshops, literature reviews, and professional societies.

Patient Population / Client Focus

Our patient population includes children and adults recovering from various conditions, such as orthopedic injuries, post-surgical rehabilitation, neurological disorders, and chronic pain. Qualifications

To qualify for this position, candidates must possess:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid State License to practice Physical Therapy.

Preferred Experience
