The Berks Community Health Center is seeking a dedicated and licensed Clinical Social Worker (LCSW) to join our team in a full-time, remote capacity. This role offers an annual salary ranging from $90,000 to $100,000, commensurate with experience. Work Environment

Key Responsibilities

• Build strong rapport with patients and their families through effective communication, actively listening and adapting to their preferred communication styles.
• Provide dynamic collaboration as a member of an interdisciplinary team, contributing to the comprehensive treatment plans that enhance patient outcomes.
• Conduct thorough assessments and screenings for behavioral health conditions such as depression, anxiety, and substance use disorders, ensuring that patients receive appropriate referrals for specialized care when necessary.
• Manage a caseload of patients, delivering timely follow-up interventions and ensuring continuity of care.
• Utilize our electronic medical record (EMR) system to document patient interactions and interventions within 48 hours for efficient delivery of care.
• Create and implement integrated care plans that prioritize the patient’s needs while coordinating communication between all parties involved in their care process.
• Safeguard patient confidentiality and comply with all regulations regarding health information protection.
• Participate actively in supervision, case conferences, and quality improvement meetings to contribute to ongoing team development and service enhancement.
• Facilitate referrals for medication management with our consulting psychiatrist when indicated, optimizing treatment outcomes for patients with complex needs.
• Actively identify community resources that support patients' goals, such as educational or vocational services, spiritual support, and self-help groups.
• Perform other tasks as necessary to support the team and its objectives.

Patient Population / Client Focus

The LCSW will focus on a diverse patient population that includes underserved communities and individuals facing homelessness. We prioritize a trauma-informed care approach, recognizing the unique experiences that can affect mental health. This role is crucial in ensuring that patients feel safe and supported while accessing the mental health resources they need.

Qualifications

• A Master’s degree in a related behavioral health field is required.
• Must possess a current LCSW license in the state of Pennsylvania.
• CPR or BLS certification is mandatory.
• A minimum of three years of experience in the behavioral health field.
• Proficiency with electronic medical records and other clinical data systems.
• Exceptional verbal and written communication skills are essential for effective patient interaction and documentation.
• A strong understanding of the principles underlying population-based care for both preventive and chronic health conditions.

Preferred Experience

• Experience working with underserved populations, including homeless individuals and those experiencing socioeconomic challenges.
• Familiarity with trauma-informed care and a social justice framework.
• Bilingual skills in English and Spanish are highly preferred.
• Demonstrated capability to manage multiple priorities and interventions effectively in a fast-paced environment.
